Forever Care
The Forever Care Pilot, led by local voluntary and community organisations (VCS), aims to better support people whose health and wellbeing needs often go unmanaged and unmet, to maximise their health and wellbeing.
This pilot is for people who experience the greatest levels of inequality.
Often they regularly use community-based and primary care services, like social prescribers, local community centres, or GP practices, but still don’t get the joined-up support they need to thrive.
Forever Care is about seeing and supporting the whole person, not just treating them through the lens of their health conditions.
Forever Care is provided in partnership with Kensington & Chelsea Social Council and London Medical Associates.
Forever Care brings together community organisations, health and care professionals to:
Identify people locally who experience inequalities and whose needs are unmet.
Create a holistic plan with the person, focused on maximising their health and wellbeing.
Coordinate support across services to meet their needs more effectively.
Provide a small budget for gap analysis and community-led grants to fill local service gaps.
